Your name will never be demanded of you if you would prefer to be anonymous; however, we do strongly encourage adding as many details of a situation as possible so the Member-At-Large can help.
Note: You do not have to be a member of the fraternity to fill this form out; in fact, we strongly encourage any non-members to let us know if there is a problem regarding how we carry ourselves as a professional fraternity.
